
Disney Cruise Line Questionnaire
On the morning of your cruise, you will receive an e-mail from DCL to complete a Health Questionnaire prior to check-in. There are 3 questions. (See Below) You will receive an email containing a link for the Mandatory Health Questionnaire early in the morning on embarkation day. You can use the link in the email or log in to the Disney Cruise Line Navigator app on the morning of your cruise to access the questionnaire. If everyone in your sailing party is healthy, you’ll be good to go to check in at the port!



Remember to have your Port Arrival Form and Travel Documents on hand. Disney Cruise Line representatives will be ready at the terminal to check you in at your Port Arrival Time. They’ll swiftly scan your paperwork and verify your citizenship documentation. While you wait for your boarding group number to be called, you can peruse the app. You’ll see your personal itinerary, rotational dining details, and a list of the onboard activities scheduled for your voyage.

If you get sick on board
The Health Center is open during daily clinic hours to provide non-emergency medical care throughout your cruise. In the event of a medical emergency, a doctor and nurse are on call 24/7.
You are responsible for all medical fees incurred in the Health Center, which will be charged automatically to your stateroom account. Additional fees for after-hours care will apply.
Medical services provided by the Health Center are considered “care outside of home country,” and insurance is not accepted on board. However, the Health Center will provide care documentation—enabling you to submit claim requests to your personal insurance carrier or other travel insurance provider, as needed.
